If you are coming to our practice for testing only, please complete the form below and bring it with you to your appointment.

TESTING PATIENT INFORMATION AND CONSENT FORM
This form is used to provide our office with the patient's personal (name, date of birth, address, etc.) and insurance information, to give your consent for receiving care, and for our office to release medical information to your primary care doctor or others, when needed.

STRESS TEST INSTRUCTIONS
This form provides information about your test.

We offer a range of in-office testing services including:

Echocardiography -- We offer echocardiography with our state-of-the-art digital echocardiography system. We provide our patients a complete non-invasive evaluation of the heart, including valvular function and assessment of ventricular function, as well as stress echo testing to evaluate for coronary artery disease.

Nuclear Cardiology -- We aso offer stress testing with nuclear imaging. This safe, non-invasive test is an excellent means of evaluating for ischemia and diagnosing coronary artery disease. In addition, exercise tolerance, cardiac function, and previous infarct status can be evaluated. Individuals with multiple cardiac risk factors such as high blood pressure, high cholesterol, or a family history of coronary artery disease may benefit from a nuclear cardiology stress test.
